Understanding Mononeuritis Multiplex: Causes, Symptoms, and Treatment Options

Mononeuritis Multiplex (MM) is a rare neurological disorder characterized by damage to two or more peripheral nerves in separate areas of the body. Unlike generalized neuropathies, MM presents with asymmetrical symptoms, often involving sudden pain, weakness, or sensory changes. This condition is frequently linked to autoimmune disorders, infections, or systemic diseases. Here, we explore its causes, symptoms, diagnosis, and treatment options.

Mononeuropathy vs Polyneuropathy

Mononeuritis Multiplex (MM) is a mononeuropathy. Mononeuropathy and polyneuropathy differ in the number and pattern of nerves they affect. Mononeuropathy involves damage to a single nerve, usually caused by injury, compression, or localized inflammation. This results in symptoms such as numbness, tingling, or weakness that are limited to a specific area of the body—for example, carpal tunnel syndrome. In contrast, polyneuropathy affects many nerves at once, often in a symmetrical pattern that starts in the feet and moves up the legs. Polyneuropathy is usually caused by systemic conditions like diabetes or certain infections, and leads to more widespread problems with sensation, movement, or autonomic function. While mononeuropathy causes isolated symptoms in one area, polyneuropathy results in broader, more generalized symptoms throughout the body.

Causes of Mononeuritis Multiplex

MM arises from conditions that disrupt nerve function through inflammation, reduced blood flow, or direct injury. Key causes include:

  • Vasculitis: Inflammation of blood vessels (e.g., polyarteritis nodosa) disrupts oxygen delivery to nerves, causing ischemia. Many forms of vasculitis are considered to be autoimmune diseases.

  • Autoimmune diseases: In addition to vasculitis, autoimmune diseases like lupus and rheumatoid arthritis are common culprits—where immune-mediated inflammation damages nerves or their blood supply.

  • Diabetes: While more often linked to symmetric neuropathy, diabetes can also trigger MM due to vascular damage.

  • Infections: Lyme disease, HIV, hepatitis, and leprosy may directly invade nerves or trigger inflammatory responses.

  • Rare causes: Amyloidosis, sarcoidosis, and certain cancers.

Diagnosing Mononeuritis Multiplex

Diagnosis involves a combination of clinical evaluation and specialized tests:

  1. Clinical exam: Assessing reflexes, muscle strength, and sensory responses.

  2. Electrodiagnostic studies:

    • Nerve conduction tests: Measure signal speed and amplitude to identify axonal damage.

    • Electromyography (EMG): Detects abnormal muscle activity caused by nerve dysfunction.

  3. Blood tests: Screen for autoimmune markers (e.g., ANCA, ANA), infections, or metabolic disorders.

  4. Biopsy: Nerve or tissue biopsies may confirm vasculitis or inflammation.

How LUX Infusion Can Help

Mononeuritis Multiplex (MM) is a complex disorder requiring prompt diagnosis and targeted treatment. IVIG therapy offers a promising option for autoimmune-driven cases, improving outcomes and quality of life.

LUX Infusion provides tailored care for MM patients undergoing IVIG therapy. As a specialty infusion pharmacy, our clinical teams have extensive training and experience in rare diseases, infusion therapies, and complex medical conditions. Knowledgeable pharmacists and care coordination staff guide each patient through the potential medication side effects and, working with the prescribing physician, help manage treatment to reduce the prevalence and severity of relapses.

We also administer in-home treatment to provide a more comfortable and convenient infusion experience. The result is safe, consistent, and compassionate treatment for our patients with nursing needs. Where appropriate, our in-home nursing team trains patients to administer their own injectable or infusible drugs, fostering more independence. After the home visit and administration, our nurses also guide patients on continuing with their therapy, help minimize or manage side effects, and answer questions that patients may have throughout their treatment plan.
